Application Process:

Applicants should be scheduled to complete their residency training in a primary care medical specialty — either Family Medicine, Internal Medicine, Pediatrics or Emergency Medicine — prior to beginning the fellowship.

Applications should include:

AMSSM application for Sports Medicine Fellowship, Current C.V., Medical School Dean's Letter, USMLE or NBMOE Board Scores, three letters of recommendation (one from Residency Program Director), and a personal statement of career goals.
